Purpose of consultancy
- This consultancy provides technical support to enhance the regional ICU/Critical care and clinical management capacity in managing critically ill patients with COVID-19. The identified Consultant supports WHO Regional Office for Africa (WHO AFRO) during the response and transition from a surge response to building a longer-term program for ICU/critical care in priority countries in the region. The Consultant is expected to coordinate the implementation of clinical management-related programs, provide technical support, conduct training as needed, engage in clinical management-related research, support optimal procurement of ICU supplies/equipment, and establish a robust regional network of critical care and clinical management for all the 47 countries of the Africa Region.
Work To Be Performed:
OUTPUT 1: Surge deployment to countries for ICU/critical care and clinical management support and capacity building in national healthcare workers in the region.
- Deliverable 1.1: Short-term deployment as ICU/critical care expert for supporting healthcare professionals caring for critically ill patients in countries
- Deliverable 1.2: Support designing a longer-term ICU/critical care training program for the region
- Deliverable 1.3: Support capacity building to prepare and respond to treatment of critically ill patients, including delivery of training
OUTPUT 2: A comprehensive report describing the need and gaps of enhancing national capacities for ICU/critical care and clinical management in the region
- Deliverables 2.1: Prepare a comprehensive report of the findings of the ICU/critical care capacities and operations in deployed countries. The report will be used by AFRO as well as by MSs to identify the gaps related to response capacities and will support information-based capacity building plans
- Deliverable 2.2: Develop a scientific manuscript to be published.
OUTPUT 3: Support WHO’s research in analyzing clinical and oxygen data and developing strategies for clinical preparedness in treating critically ill patients in the region.
- Deliverable 3.1: Support AFRO in collecting clinical and oxygen data and analyzing the collected data in the region.
- Deliverable 3.2: Support AFRO in developing clinical management and oxygen scale-up strategies based on the findings from research data.

Educational Qualifications
- Essential: A university degree in the Medicine.
Desirable:
- Qualification in specialized Medicine (critical care, pulmonary medicine, infectious disease, emergency medicine or other relevant areas), training and education, and/or other related areas.
Experience
Essential
- At least five years’ clinical experience working on the clinical management of respiratory diseases and critically ill patients. The incumbent is expected to advise countries and help design an ICU/critical care program for healthcare professionals.
Desirable:
- Relevant experience at the international levels, in the case management and clinical care activities/infectious diseases in an outbreak, disaster or humanitarian emergency. Working experience with WHO or UN agencies, sound knowledge in research and specialty in infectious diseases or respiratory diseases are assets.
Skills/Knowledge
- Proven understanding of the management of acute and urgent public health responses, including infectious disease outbreaks;
- Experience in the outbreak and emergency response;

Emergency Roster of Consultants – Epidemiologist
Purpose of consultancy
- In the context of the WHO health emergencies Incident Management System (IMS), at the country level, the incumbent will implement and coordinate WHO and health cluster strategies for the monitoring and implementation of public health surveillance and response programmes with a special focus on early detection, verification and rapid response to epidemics and other public health incidents related to the health emergencies. The incumbent will provide technical guidance to the Ministry of Health and other partners in strengthening surveillance and response for priority diseases, conditions and events in the context of Integrated Disease Surveillance and Response (IDSR) and for capacity building of national experts, in compliance with the International Health Regulations (IHR 2005).
Assessment design and implementation will use different tools, including but not limited to:
- Multi-cluster/sector Initial Rapid Assessment (MIRA)
- Humanitarian Emergency Setting Perceived Needs Scale (HESPER)
- Standardized Monitoring and Assessment of Relief and Transitions (SMART Nutritional and retrospective mortality survey)
- Rapid risk assessment (RRA) of acute public health events
- Public Health Situation Analysis of humanitarian affairs.
- Early warning, alert and response (EWAR) to epidemic-prone diseases (infectious disease surveillance)
Specific monitoring will be carried out using the following methods:
- Health Resources and Services Availability Monitoring System (HeRAMS)
- Response mapping using the 3/4Ws matrix
- Health Cluster Performance Monitoring WHO Emergency Response Framework monitoring
- Needs Assessments
- Contribution to Humanitarian Needs Overview (HNO) and Humanitarian Response Plans (HRPs)

Deliverables
- During deployment, the duty station may change, and duties may be modified based upon the technical needs of the Programme:
- Conduct real-time analysis of outbreak intelligence data, with other team members, for epidemic intelligence; guide team members on data quality improvement; use data for monitoring trends, ALERT and ACTION thresholds (triggers), and methodologies for performing data quality assessments (DQA).
- Oversee data entry, data validation, analysis and reporting of assessments and surveys. Synergize data received from multiple sources, including health information systems, death registries, vertical reporting programmes, community-based surveillance and IDSR.
- Coordinate the establishment and functionality of Rapid Response Teams (RRTs), early warning systems (including event-based and indicator-based surveillance) for epidemic and pandemic-prone diseases for early detection and response to public health events,
- Contribute to preparing other WHO and health cluster communication products (situation reports, health cluster bulletins, donors’ alerts, and other ad-hoc reports).
- Design, in cooperation with technical partners, surveillance guidelines, protocols, surveys and monitoring/surveillance systems for detection, verification and response to public health incidents at the national level; ensuring efficiency of data collection, correlation, analysis, interpretation, timely reporting and coordinate the work of a network of sub-regional monitors, as well as provide technical guidance to the epidemiologists assigned, as required.
- Represent WHO in coordination meeting with partners involved in epidemiology, surveillance and monitoring.
- Develop appropriate training plans for building the core surveillance, alert and response national capacities following IHR 2005. Oversee the capacity building at national, subnational and health facility levels.
- Lead the development, consolidation and maintenance of a country-specific and event-specific repository of information (at the highest geographic resolution possible) on core public health indicators (e.g. crude and under-five mortality rates, prevalence of malnutrition, vaccination coverage); updated population estimates (resident, displaced, refugees), standards indicators and values used by partners for monitoring needs and response; vulnerability mapping; nutrition, mortality and vaccination coverage survey reports; health needs assessment reports.
- Perform any other incident-specific related duties, as required by the functional supervisor.

Emergency Roster of Consultants – Field Coordinator
Purpose of consultancy
- The purpose of the roster is to provide short-term support during acute events for up to three months which may be extended up to six months to one year depending on operational needs. Placement on the roster entails a general commitment from the external candidate to deploy within 1 to 2 weeks from when a request for availability is made.
During deployment, the duty station may change, and duties may be modified based on the technical needs of the Programme:
- Coordinate WHO’s incident management system at the Sub-national level, assess and reassess the emergency, determine strategies, set objectives appropriate to the level of the required response, prioritizing filling health service delivery gaps, develop an operational planning cycle to reach health sector performance standards and approve the incident action plan (IAP) ensuring fulfilment of WHO’s critical functions.
- Hold regular coordination meetings with operational partners working at the Sub-National level, building when possible on existing coordination fora in collaboration with the subnational Health Cluster Coordinator
- Ensure the implementation of the IMS, ensuring synergies and smooth functioning between the operational and technical components, oversee the monitoring and evaluation of the operations, and take corrective actions as necessary.
- Manage and supervise all WHO staff under his/her jurisdiction
- Participate in joint risk assessment needs analysis and ensure that humanitarian response needs (human resources, logistics, financial, security etc) are identified by planning and jointly coordinating with partners rapid assessments.
- Identify and make contact with local partners involved in the response and existing Sub-National coordination mechanisms, including Sub-National health authorities, international organisations and civil society – to advocate for operational gap-filling
- Guide the respective sub-national level’s capacities with respect to emergency preparedness, response and recovery; ensure the participation of the country office in the joint risk assessment process of any suspected or actual emergency event.
- Build external relations to represent WHO and its positions in negotiations in relevant inter-agency and international for a involved with the incident management system at the country level. Immediate actions:
- Lead and Manage the Health Emergency Operations at the subnational level and provide technical advice to other agencies, emphasising the coordination and consolidation of health agencies and health cluster activities. Coordinate overall incident health-related activities to ensure the concerted approach of WHO in all required functional areas as per incident management structure.
- Support national and local health authorities and partner with UN agencies, international, governmental and nongovernmental organizations/institutions, in respect of the mobilization and coordination of resources, identify needs and funding gaps; encourage donors to fund the overall health sector response to affected populations.
- In cooperation with other levels of response, authorize the release of related information, communications, advocacy products and statements to internal and external partners as well as to the public. Provide information to/briefing of senior and elected officials as required, manage sensitive issues arising from the incident.
- Determines WHO response team staffing needs and ensures deployment arrangements and the mobilization of the required human resources. Provide leadership, motivation, guidance, and performance management of staff and experts, build dynamic and efficient teams to bolster WHO’s incident management operations, take concrete measures to ensure the safety and wellbeing of all WHO responders.
- Collect information from all WHO technical experts, Regional Health bureau, other UN agencies and operational partners at Sub-National partners on Who, Where, since and until When, doing What, and regularly report to the National coordinator for feeding into the database managed by WHO. Provide consolidated feedback to all partners
- Assess and monitor the response activities related to humanitarian crisis in line with the Sub-National operational plan, including increasing access to basic lifesaving services, outbreak preparedness and response, Disease outbreak surveillance, social mobilisation and community engagement, case management; IPC, laboratory, communication and information management.
- Establish daily contact mechanisms with each affected area (hotspot) to monitor the situation and facilitate the implementation of required actions. Summarise and provide daily Sub-National updates to the Incident Manager using a standard reporting template and ensure the following of agreed actions.
- Promote adherence of standards and best practices by all response personnel, taking into account the need for local adaptation. Promote use of the available response guidelines to ensure the application of common approaches, tools and standards.
- Oversee demobilisation of the Operations Centre and restocking of supplies, as appropriate.
- Ensure that after-action activities are coordinated and completed, including collection of all event documentation and conduct of staff debriefings to identify accomplishments, response and improvement issues. Emergency Roster of Consultants – Risk Communication and Community Engagement and Infodemics
Purpose of consultancy
- The Consultant will work under the overall leadership of the HOTE pillar lead, direct supervision of the RCCE sub pillar lead, and coordination with other personnel at AFRO and WCOs. The Consultant will liaise between RCCE Technical Leads and country office (CO) staff to ensure communication, coordinate and organize meetings, and perform other knowledge management tasks necessary to move activities forward. The Consultant will also co-facilitate the regional training and special country training as needed.
Deliverables
The Consultant with support AFRO in the following key tasks:
- Establishing a strong/cohesive RCCE partner coordination at global, regional and country levels for a more effective response;
- Communicating science information/recommendations promptly that address critical risks and counter misinformation;
- Accelerating priority research and innovation in social sciences to support the implementation of public health measures and to ensure participation of at-risk and affected communities; and
- Enhancing country-level capacity to roll out effective and coordinated RCCE approaches through the identification of capacity needs.
Detailed Roles:
- Strengthen the capacity of national and local media and communications organizations to communicate accurately about COVID-19 and counter misinformation.
- Facilitate multi-sectoral engagement to mitigate the effects of socio-economic-political drivers of COVID-19 and contribute to the resilience of families and communities;
- Integrate RCCE into the design, implementation and monitoring for all COVID-19 response efforts to avoid duplication and gaps and to maximize the sharing of RCCE resources, standards, indicators, evidence and expertise;
- Establish mentoring systems to enable those at national and subnational levels to access technical support in an accessible and timely manner, helping improve the quality of RCCE approaches;
- Develop robust systems and processes around knowledge management and the documentation of approaches, lessons and best practices;
- Track trending questions by demographic/location/ language/gender/age etc. Map these against available content to respond to those questions, as a basis for gap and trend analysis, prioritization and monitoring;
- Develop strategies to prevent and address stigma and discrimination, overcome pandemic fatigue, and build and maintain trust;
- Coordinate efforts to manage the infodemic, build digital and health literacy and mitigate the impact.
- Balance digital engagement with safe and appropriately resourced in-person engagement to ensure that vulnerable groups and those who cannot access digital channels are not left behind;
- Invest in community ownership approaches, processes and systems to enable community-centred approaches to roll-out testing, treatments and vaccines;
- Provide formal and informal community health workforce and local actors with knowledge, engagement and interpersonal communication skills, supplies and tools;

- Identify and strengthen the systems needed for sustainable long-term community engagement and empowerment, to support communities beyond COVID-19.
