Care Manager

Job Objective/Purpose

  • Monitor ongoing quality, productivity, efficiency and cost effectiveness of services and liaison person with third party payers.

Key Responsibilities

  • Conducts daily in patient rounds of the specific scheme beneficiaries admitted in the hospital. To ensure timely and appropriate admission, diagnosis, and treatment, care of patients and advise the hospital and the corporate accordingly.
  • Ensure only authentic pre-authorizations are issued for clients seeking medical services in collaboration with the clinical medical team.
  • Draw monthly reports for specific corporate clients inclusive of trends,analysis, market intelligence for improved business development
  • Liaise daily with other concerned units especially credit control and clinical department to seek amicable solutions to problems related to our billing and invoicing system.
  • Conduct a regular review of the case process to identify gaps to enhance profitability as well as efficiency and effectiveness in the case management process.
  • Enhance customer experience – establish and maintain good relationships with external and internal stakeholders such as underwriter’s, brokers, agents, and policy holders.
  • Source, analyze and provide relevant information to the management for further decision making.
  • Support development of a preventative care program through health talks, wellness, that is a key value proposition to the hospital business chronic disease management program.
  • Prepare the business reports on a daily or weekly basis as and when need be.
  • Ensure incidents are documented by respective staff and discussed with the head of department at all times.
  • Attend inter-phasing departmental meetings and convene meetings every fortnight to identify gaps in care co-ordination in-order to improve services.
  • Collaborate and co-ordinate with other departments, Consultants and other Clinicians to improve overall health service delivery in the department.
  • Conduct continuous impact assessments and identified the most appropriate strategies to bridge established gaps in order to enhance efficiency in service delivery.
  • Induction of new staff working in inter phasing departments as per the departmental/unit procedure manuals.
  • Continuously improve on the hospital business NPS 
  • Any other duty as assigned by the supervisor in line with the job description.

Personal Specifications

  •  A BScN in nursing and administrative training 
  •  Valid Kenyan nursing practice license.
  •  3 years proven experience as a care manager.
  •  In-depth knowledge of health and safety guidelines and procedures

Health Promotion Coordinator

Job Objective/Purpose

  • Utilize a public health approach to create, provide, and coordinate wellness and health promotion, health outreach, prevention programs and services. To deliver successful schools health promotion and learner’s rescue program.

Key Responsibilities

  • To establish successful and functional school health clubs in all schools involved in the program.
  • To establish successful and functional Avenue Learners emergency rescue subscription service.
  • To grow the number of schools involved in the school’s health promotion program.
  • To grow the number of schools and lives subscribing to the Avenue Learners emergency rescue service.
  • To leverage on the school’s health promotions program to create commercial opportunities and gains for Avenue Healthcare.
  • Coordinate the planning and delivery of wellness programs including health awareness and educational campaigns, behavior change programs, health assessments and health screenings at all designated client locations.
  • Understand and be able to evaluate the application of health communications, health behavioral theories and models within all clinical health promotion settings including coaching.

Personal Specifications

  •  Bachelor’s degree in nursing, health promotion, public health, social work, nutrition, or related field of study preferred. 
  •  Experience in health promotion, wellness programs or health education.
  •  Relevant professional experience in human services field required

Human Resource Assistant

Job Objective/Purpose

  • Support the administration and implementation of Human Resources policies, processes, and procedures to achieve organizational objectives.

Key Responsibilities

  • Support the recruitment and induction processes and ensure new staff joining the organization go through the entire recruitment and on boarding cycle and that all documentation required is on file.
  • Carry out timely reference checking for all new staff and follow up with HROs to ensure timely confirmations at probation period end for new employees.

Job Objective/Purpose

Key Responsibilities

  • Ensure proper inventory of the available equipment on a daily basis and ensure that they are in good working condition. Report any malfunction to Bio-Medical Technician/Unit Manager.
  • Ensure the unit is financially viable and demonstrate growth through;
    • Offering quality care to patients and ensuring good working relationship with our stakeholders
    • Ensure financial objectives are met by managing costs within budget, timely and accurate billing of drugs and services. Ensure an invoice rejection rate of < 3% by ensuring that proper pre-authorisations and documentation has been done.

Unit Manager – Physiotherapy

Job Objective/Purpose

  • To improve the medical condition of a patient by using a variety of treatments to alleviate pain and restore function or, in the case of permanent injury or disease, to lessen the effects of any dysfunction.

Key Responsibilities

  • Assess patients’ total physical function and mobility. 
  • Design and implement a treatment plan based on the patient assessment.
    • Ensure that the treatment plans, daily, weekly or other scheduled plan which include using specialized techniques. 
  • Ensure patients safety in a clean environment 
  • Ensure patients are educated on their needs especially those who need long term rehabilitation strategy
