Paediatric Nurse, Home Health Services
Position Summary
The Paediatric Home Health nurse aims to provide care that is compassionate, safe, ethical, patient- and family-centered, culturally congruent and evidence-based to enhance quality of life, augment accessibility and continuity of care, and improve patient outcome. The position holder will be responsible for providing community based services which include comprehensive nursing assessment, patient advocacy, exquisite care-coordination of medical care teams, routine medication administration and supportive care measures under clinical supervision to paediatric patients.
Responsibilities:
- Perform comprehensive patient and environmental assessments related to home health needs
- Develop care plan in consultation with clinician
- Coordinate and implement care delivery while anticipating patient care needs across the continuum, utilizing nursing process (assessment, diagnosis, planning, implementation and evaluation)
- Provide nursing care, administer medication and perform routine clinical procedures (including vital signs, I/V cannulation, access ports, IV drug/fluid administration, phlebotomy, collection and handling of specimens, etc.) according to program policies within the prescribed plan, in the home setting
- Monitor the patient’s physiological, physical and emotional response to treatment and identify patterns and trends. Able to pick up early warning signs and seek immediate support
- Evaluates patient’s response to plan of care and works in collaboration with the primary clinician to make any needed adjustments
- Support the patient and family (including psychosocial support) through disease progression, symptom management, advanced care planning and life closure, as appropriate
- Identifies learning needs of the patient and families and educates them by adapting to patient and family learning style utilizing the teach-back method.
- Fulfil safety and regulatory requirements: Perform initial and routine safety checks to ensure compliance and to assess and maintain a safe environment for patients, families, and staff in accordance with regulatory and institutional health standards.
- Ensure appropriate handling and disposal of biological and hazardous specimens/materials
- Oversee inventory management and facilitate maintenance of clinical supplies
- Ensure all near-misses, accidents, incidents and complaints are reported and to assist in the timely investigation and closure of same.
- Manage and maintain patient records for each visit
Requirements:
- KRCHN/BScN from a recognized school of nursing or University
- Current licensure with the Nursing Council of Kenya
- Certification in European Paediatric Advanced Life Support(EPLS)
- Minimum requirement of 3 years’ continuous clinical experience in medical surgical Nursing. Experience in oncology, home care and/or palliative care is preferred
Patient Services Coordinator, Outreach Clinic Parkplace
Responsibilities:
- Provide outstanding customer services to patients, public and AKU staff and ensure inquiries and complaints to the department are addressed expeditiously.
- Consistently always provide full coverage at the front desk by clocking in and out in the HR time management software.
- Apply all functional computer applications to process daily front office transactions at compliance level standards within 30 days of introduction.
- As a patient services coordinator, maintain a manual invoice and receipt process in the event of a computer system malfunction and upon resumption of computer system., update data and records in the computer system.
- Coordinate Dental Appointments on both dental software and Meditech. Manage Dental Preauthorizations
- Ensure patients are registered accurately, following two patient identifiers –Ak number and patient full names.
- Beware of the down time and data protection procedure in the event of a system failure.
- Account for and balance all monies collected from patients on daily basis then bank the money or hand over the monies to clinic in charge for safe keeping at the end of a work shift awaiting banking the following day. To know the Zero Tolerance Policy.
- Look in the computer system and verify and confirm credit services that are applicable to corporate employees as per the account entitlement details.
- Ensure the Pre-Authorizations are duly completed by the Physician’s and sent to corporations for approval within 24hours.
- Liaise with clinic in charge, manager, and patient’s insurance company regarding any uncertain approval of such cases.
- Check and verify patient data in the computer system and bill the correct patient for the right services and collect the right amount of payment for the services.
- Verity that all benefits for corporate employees are billed as per SMART CARD entitlement.
- Ensuring Patient biometric registration is done.
- Assist other patient services staff when the need arises to foster overall efficiency of the department and deliver timely patient care.
- Creation, Storage, retrieval, and general maintenance of patients’ records. Scanning of medical documents to patient records. (EDMS)
- Answers both internal and external phone calls promptly and respond appropriately. Communicate messages promptly and accurately to concerned Persons to help in the maintenance of smooth organization of patient care. Respond to Talk to Us Emails regarding patient appointments efficiently
- Re confirm/Updates contact numbers of patients. In addition, ensure realistic appointments are booked and rescheduling is done on a timely basis. Rescheduling of appointments by the physicians must be communicated to the patients on time by the staff at the consulting clinics.
- Complete any other duties that may be required by the supervisor or manager.
Requirements:
- Diploma in Front Office Administration, Business Administration or an equivalent qualification deemed applicable by AKUH
- 2 years working in a busy front office preferably in a service-oriented industry.
- Proficient in computers and a hospital billing system e.g Meditech
- Proficient in handling, counting and recording cash & cash transactions.
Clinical Nurse, Maternity
Responsibilities:
- Complete initial assessment and re-assessment of obstetric mothers and new-born babies to identify their care needs
- Document all assessment data, investigation results, care given, discussions with the multidisciplinary team, family/patient interactions in an accurate, comprehensive and timely manner
- Develops and implements prepartum, intrapartum, post-partum and new born care plan
- Proficient in Management of labour, delivery and third stage
- Ability to perform and repair episiotomy
- Ability to repair first and second degree tears
- Carry out and interpret Cardiotocography (CTG) and Non Stress Test (NST) and act on findings
- Administers safely and is knowledgeable about medications routinely used in obstetrics
- Is able to recognize obstetric and neonatal emergencies and involve multidisciplinary team as appropriate
- Initiates timely initial immunizations for new borns as per guidelines
- Proficiency in caring for mothers’ post- surgical procedures
- Perform timely safety checks on equipment as per policy guidelines
Requirements:
- KRCHN/KRM/BScN from a recognized school of Nursing or University
- Current Licensure with the Nursing council of Kenya
- Certificate in Basic Life Support (BLS)
- Certification in advanced life support in obstetrics ( ALSO)
- Minimum of three (3) years’ experience in an acute care hospital.
- Demonstrate evidence based knowledge of current practices in Obstetric nursing
